Personal Action Toward Health (PATH)

The National Kidney Foundation of Michigan (NKFM) is hosting a free health workshop in Rochester Hills from January 19 to February 23 to help individuals manage long-term health conditions. The Personal Action Toward Health (PATH) class will meet on Saturdays from 10:00am to 12:30pm at Barclay Physical Therapy, located at 555 Barclay Circle, Suite 110.

The PATH workshop provides information and skills to adults with chronic health conditions such as arthritis, heart disease, chronic kidney disease, diabetes, bronchitis, asthma, and depression. Workshops are conducted by trained leaders who hold informal, small group discussions and provide easy-to-understand course materials. Facilitators discuss ways to reduce pain and stress, cope with fatigue, use medications wisely, and benefit from physical activity. Participants also learn about self-help devices that can enhance daily activities. They are encouraged to practice the acquired skills by making weekly action plans and helping each other achieve their goals.

The PATH program is appropriate for people who are newly diagnosed as well as those who have been managing a chronic condition for years. It has been proven in studies to reduce symptoms and physician visits as well as increase self-management abilities and communication with doctors.
